REMEMBER THE CARER FOR ALLIED HEALTH & COMMUNITY ORGANIZATIONS
Practical, Non-Medical Support for Dementia & Cognitive Change
Working Together
to Support Dementia & Cognitive Change in the Community
Remember The Carer partners with allied health professionals and community organisations to provide non-clinical, human-centred support for individuals living with dementia or cognitive change, as well as the families and carers who support them.
Alongside our Dementia Care Specialist, our multidisciplinary, non-clinical team — including Certified Neuroscience Coaches, Wellbeing Alignment Mentors, and internationally trained Dementia Practitioners — works in collaboration with your services, not in place of them.
Our role is to complement and extend care, helping clients and families better understand cognitive change, navigate daily challenges, and remain supported between appointments, programs, and community touchpoints.
Supporting Care Beyond Appointments & Programs
Allied health and community organisations see the same challenge every day:
needs are complex, time is limited, and much of what destabilises care happens outside formal sessions.
We provide:
-
Post-diagnostic and post-assessment education
-
Practical dementia and cognitive-change understanding
-
Carer and family support
-
Neuroscience-informed wellbeing coaching
-
Emotional and practical guidance that reduces overwhelm and crisis escalation
This support helps clients and carers stay more regulated, informed, and engaged — which in turn strengthens the effectiveness of your services.
A Collaborative Referral & Care Partnership
Partnering with Remember The Carer enables:
-
Mutual referral pathways that keep care connected
-
Reduced pressure on allied health and community teams
-
Better-supported clients between appointments
-
Improved engagement, continuity, and sustainability of care
-
Reduced burnout for carers — and for professionals
We work alongside your organisation to ensure clients are supported across the full care journey, not just during formal sessions.
Serving Your Organisation & Your Staff
In addition to client and carer support, we also serve your organisation directly through:
-
Dementia and cognitive-change awareness education
-
In-service training and information sessions for staff
-
Practical tools to support conversations with clients and families
-
Resources that help staff feel more confident and supported in their roles
This builds shared language, confidence, and consistency across care.
Flexible, Accessible Support
Appointments are available 7 days per week, including evenings and weekends, via secure telehealth — allowing clients and carers to engage from their own environment, at a time that works for them.
Private Partner Portal Access
Partner organisations receive access to our Private Provider Portal, offering:
-
Tailored resources for allied health and community teams
-
Referral guidance and collaboration tools
-
Staff education opportunities
-
Updates and shared resources to support your clients
